Join a Club

Club Sport teams are available to all enrolled full-time Colorado College undergraduate and graduate students. Faculty, Staff, Alumni, and community members are not eligible. The Colorado College Club Sports Program welcomes and encourages the widest possible participation in sports regardless of ability or experience. Club Sport teams may host tryouts at the beginning of their competitive season. All Club Sport teams are student led. Contact a Club Sport Captain, found on the Quick Contacts webpage, to learn more about each Club including tryout dates, practice, and competition schedules.

The following pre-participation requirements must be completed to become a member of a Club Sport and to be eligible to participate in all Club Sport sanctioned events including games and competitions. Contact the Campus Recreation Athletic Trainer for questions regarding pre-participation requirements.

Pre-Participation Forms

All new Club Sport student-athletes must complete the following forms prior to their first game or competition. The following forms can be completed via the Healthy Roster app. All rostered Club Sports student-athletes will be granted access to Healthy Roster once Club Captains submit their Squad List. View the tutorial videos below under the Healthy Roster section to learn how to access and upload your forms.

  • Assumption of Risk and Liability

  • Consent to Treat
  • Emergency Medical Form
  • Health History Form (To be completed by Returners only)

Baseline Concussion Test

The Baseline Concussion Test is administered by the Campus Recreation Athletic Trainer and must be completed prior to first game or competition. If a Club Sport student-athlete sustains a concussion, a new baseline must be administered. Baseline Concussion Testing is scheduled by Club Sport Captains with the Campus Recreation Athletic Trainer.

Physicals

All rostered Club Sport student-athletes must have a Pre-Participation Physical Exam on file with the Campus Recreation Athletic Trainer prior to the Club team's first competition. Physicals can be completed at the Student Health Center, with your Primary Care Provider, or at an off-campus clinic. All Physicals must be completed using the form below. All Physicals should be uploaded to Healthy Roster upon completion.

Healthy Roster

Healthy Roster is the electronic medical record system that all Club Sport student-athletes will have access to via the mobile application and desktop. All new Club Sport student-athletes are required to register an account. Club Sport student-athletes will utilize Healthy Roster to view their medical records, schedule appointments with the Campus Recreation Athletic Trainer, and complete pre-participation forms. The following links provide video tutorials of common tools Campus Recreation student-athletes will utilize within the Healthy Roster app.
